{"id":1741,"date":"2017-08-09T17:56:07","date_gmt":"2017-08-09T15:56:07","guid":{"rendered":"http:\/\/tecnodiabetes.ai2.upv.es\/?p=1741"},"modified":"2023-02-15T17:46:32","modified_gmt":"2023-02-15T16:46:32","slug":"pancreas-artificial-logica-difusa","status":"publish","type":"post","link":"https:\/\/tecnodiabetes.ai2.upv.es\/index.php\/pancreas-artificial-logica-difusa\/","title":{"rendered":"\u00bfC\u00f3mo controla la glucosa un p\u00e1ncreas artificial?: la l\u00f3gica difusa"},"content":{"rendered":"<p>En esta tercera entrega sobre algoritmos de control para el p\u00e1ncreas artificial queremos introducir al control basado en <strong>l\u00f3gica difusa<\/strong> (<em>fuzzy logic controller<\/em> &#8211; FLC). Est\u00e1 representado por el sistema MD-Logic (Glucositter), desarrollado por DreaMed Diabetes Ltd y licenciado a Medtronic en 2015. Otro sistema basado en l\u00f3gica difusa es el desarrollado por la empresa Dose Safety.<\/p>\n<blockquote><p>La l\u00f3gica difusa (o borrosa, como tambi\u00e9n se traduce del t\u00e9rmino ingl\u00e9s <em>fuzzy<\/em>) es una formalizaci\u00f3n matem\u00e1tica que permite representar el conocimiento de un m\u00e9dico experto en forma de <strong>reglas del tipo SI&#8230;ENTONCES&#8230; expresadas en lenguaje natural<\/strong>, y realizar inferencias a partir de la misma para computar la infusi\u00f3n de insulina m\u00e1s adecuada.<\/p><\/blockquote>\n<p>En palabras simples: el sistema \u201cemula el razonamiento del m\u00e9dico experto\u201d.<\/p>\n<p>Los sistemas basados en l\u00f3gica difusa tienen su raz\u00f3n de ser en sistemas de alta complejidad donde no es posible disponer de buenos modelos de comportamiento, pero donde, en base a la experiencia, un experto es capaz de realizar un buen control del mismo. El control de glucosa es un ejemplo claro de este tipo de sistema.<\/p>\n<p>As\u00ed pues, un sistema basado en l\u00f3gica difusa consta de un conjunto de reglas condicionales como, por ejemplo<\/p>\n<p>SI (glucosa_actual es <em>baja<\/em>) ENTONCES (bolo es <em>0%<\/em>)<br \/>\nSI (glucosa_actual es <em>normal<\/em>) y (tendencia_futura_glucosa es <em>descendiente<\/em>) ENTONCES (basal es <em>-60%<\/em>)<br \/>\nSI (glucosa_actual es <em>normal_alta<\/em>) y (glucosa_futura es <em>normal_alta<\/em>) ENTONCES (basal es <em>+60%<\/em>)<\/p>\n<p>Adicionalmente a la base de reglas, es preciso definir el significado de cada una de las etiquetas ling\u00fc\u00edsticas empleadas. \u00bfQu\u00e9 significa que la glucosa actual es <em>baja<\/em>, <em>normal<\/em>, <em>normal_alta<\/em>, etc.? Para ello se recurre a los \u201cconjuntos difusos\u201d. Si hablamos del intervalo de glucosa [90, 140] mg\/dL, podemos decir con seguridad, atendiendo al m\u00e9dico experto, que la glucosa es <em>normal<\/em>. Pero, \u00bfsignifica eso que un valor de glucosa de 89 mg\/dL o de 140.5 mg\/dL no es en absoluto normal? As\u00ed como, dado un intervalo de valores o conjunto est\u00e1ndar, un valor num\u00e9rico determinado <em>pertenece o no<\/em> a ese intervalo (l\u00f3gica binaria), un conjunto difuso no tiene fronteras bien definidas. Es decir, dir\u00edamos que los valores 89 o 140.5 mg\/dL est\u00e1n muy cerca de ser normal. Matem\u00e1ticamente esto se describe asignando a cada valor num\u00e9rico un grado de pertenencia al conjunto, definido entre 0 (es con certeza <em>no normal<\/em>) y 1 (es con certeza <em>normal<\/em>). Valores intermedios denotan diferentes grados de \u201cnormalidad\u201d. <\/p>\n<p>Finalmente, el proceso de \u201cinferencia difusa\u201d permitir\u00e1, a partir de la medida del monitor continuo de glucosa, tendencias, predicciones futuras, etc., obtener para cada regla en qu\u00e9 grado se cumple su antecedente (la parte SI), definiendo de forma correspondiente en qu\u00e9 grado contribuye el consecuente de dicha regla (la parte ENTONCES) al c\u00f3mputo final de la infusi\u00f3n de insulina.<\/p>\n<p>La pr\u00f3xima generaci\u00f3n de p\u00e1ncreas artificial de Medtronic, el sistema 690G, incorporar\u00e1 la tecnolog\u00eda de l\u00f3gica difusa. Seg\u00fan la informaci\u00f3n registrada en <a href=\"https:\/\/clinicaltrials.gov\/ct2\/show\/NCT03040414\" target=\"_blank\" rel=\"noopener\">clinicaltrials.gov<\/a>, se espera que en diciembre de 2017 se inicie un estudio cl\u00ednico con 100 sujetos entre 14 y 30 a\u00f1os comparando las prestaciones de la 670G y la 690G, especialmente en las comidas.
