Ya conoc\u00e9is el algoritmo PID, en el que se basa el sistema Minimed 670G de Medtronic. Hoy queremos introduciros al <strong>algoritmo MPC<\/strong>, utilizado por un gran n\u00famero de sistemas como el Florence, de la Universidad de Cambridge, el inControl AP de TypeZero Technologies, el Horizon Artificial Pancreas de Insulet y el iLet de Beta Bionics. <\/p>\n<blockquote><p>\nLas siglas MPC significan <strong>Control Predictivo basado en Modelo<\/strong> (Model Predictive Control en ingl\u00e9s) y definen la esencia de este algoritmo: la optimizaci\u00f3n de la infusi\u00f3n de insulina a partir de predicciones de la evoluci\u00f3n futura de la glucemia.<\/p><\/blockquote>\n<p>El control MPC (que en realidad agrupa a una familia de algoritmos) surge a finales de los a\u00f1os 70 en la industria del petr\u00f3leo donde hab\u00eda que resolver problemas de control complejos con multitud de variables y restricciones.<\/p>\n<p>A diferencia del control PID, el control MPC predice c\u00f3mo la glucemia va a evolucionar en los pr\u00f3ximos minutos (valores de 45-60 minutos son habituales) y calcula, a partir de dicha predicci\u00f3n, el perfil \u00f3ptimo de infusi\u00f3n de insulina que habr\u00eda que aplicar para que el \u201ccoste\u201d del control durante ese tiempo sea lo m\u00e1s peque\u00f1o posible. Tras ello, se aplica la infusi\u00f3n que toca ahora y se repite el proceso. Esto plantea dos preguntas: \u00bfC\u00f3mo puede predecirse la glucosa? \u00bfC\u00f3mo se mide el \u201ccoste\u201d del control? <\/p>\n<p>Para ser capaz de predecir la evoluci\u00f3n de la glucosa es preciso disponer de un modelo matem\u00e1tico (conjunto de ecuaciones) que aproxime c\u00f3mo responde el paciente. Existen sistemas que incluyen modelos que describen la fisiolog\u00eda (como el sistema Florence). Otros usan modelos que tratan de explicar \u00fanicamente datos experimentales, sin importar los procesos fisiol\u00f3gicos que los han producido. En ambos casos es necesario un ajuste del modelo a partir de datos f\u00e1cilmente disponibles del paciente.<\/p>\n<p>Respecto a c\u00f3mo medir el \u201ccoste\u201d del control, no existe una definici\u00f3n \u00fanica y, junto con el modelo, es uno de los elementos distintivos de los diferentes sistemas MPC. En l\u00edneas generales, se pretende que la predicci\u00f3n de la evoluci\u00f3n de la glucosa est\u00e9 cerca del objetivo gluc\u00e9mico, penalizando infusiones elevadas de insulina. Es decir, cuanto m\u00e1s cerca est\u00e9 la predicci\u00f3n del objetivo con la menor insulina posible, mejor. Adem\u00e1s, una ventaja del control MPC es la facilidad de considerar restricciones, como por ejemplo l\u00edmites en la insulina a bordo, durante el proceso de optimizaci\u00f3n.<\/p>\n<p>En un estudio reciente en entorno controlado comparando los algoritmos MPC y PID se demonstr\u00f3 que ambas estrategias son seguras y eficientes, igualando o superando el MPC al PID. Sin embargo, este tipo de comparativas son siempre dif\u00edciles y hay que extrapolar con cuidado ya que son espec\u00edficas de los dise\u00f1os realizados en dicho estudio.<\/p>\n<p>\u00bfQuieres conocer m\u00e1s?
