Aunque los sistemas de administraci\u00f3n autom\u00e1tica de insulina (AID) han aliviado esta tarea, los pacientes a\u00fan necesitan contar carbohidratos y administrar bolos de insulina en las comidas, lo que hace que los sistemas sean susceptibles a errores humanos, lo cual podr\u00eda tener graves consecuencias para su salud. Las terapias adyuvantes, como los agonistas de receptores de GLP-1, intentan mitigar estos riesgos al reducir el impacto de las comidas en los niveles de glucosa en sangre. Estos f\u00e1rmacos se recetan a personas con diabetes tipo 2, pero actualmente se est\u00e1 evaluando su uso para DT1. Los simuladores son una de las herramientas esenciales para el desarrollo de sistemas AID y la base de los gemelos digitales, permitiendo probar y validar algoritmos de control evitando riesgos a los pacientes. Sin embargo, ning\u00fan simulador de DT1 incluye descripciones de los efectos de GLP-1.<\/p>\n<p>El proyecto T1SIA desarrollar\u00e1 un simulador de DT1 que incluya un modelo matem\u00e1tico de la din\u00e1mica de los agonistas de receptores de GLP-1. Este proyecto de dos a\u00f1os estar\u00e1 apoyado por la experiencia de la Universidad de Berna y la Universidad Polit\u00e9cnica de Valencia. El primer a\u00f1o en Berna se centrar\u00e1 en el desarrollo de un modelo matem\u00e1tico de GLP-1, supervisado por un experto en sistemas de AID y un cl\u00ednico experto en terapias adyuvantes. Durante el segundo a\u00f1o en Valencia, el modelo se integrar\u00e1 en un simulador de DT1 junto con una cohorte de pacientes virtuales bajo la supervisi\u00f3n de un experto en tecnolog\u00eda diab\u00e9tica. Este es el caso de la diabetes, en la que factores como las diferencias en el estilo de vida (dieta y ejercicio), las hormonas sexuales y la composici\u00f3n corporal, producen diferencias en la regulaci\u00f3n de la glucosa entre mujeres y hombres, lo que lleva a un peor control gluc\u00e9mico en las mujeres. En consecuencia, las mujeres con diabetes tipo 1 tienen un 37% m\u00e1s de riesgo de muerte y el doble de riesgo de sufrir eventos vasculares que los hombres. Por otra parte, algunos estudios han puesto de manifiesto la influencia de la fase del ciclo menstrual en el control de la glucosa en mujeres f\u00e9rtiles, as\u00ed como cambios en la sensibilidad a la insulina entre las fases folicular y l\u00fatea en mujeres con diabetes de tipo 1.\n\nA pesar de los avances tecnol\u00f3gicos del mercado, como el p\u00e1ncreas artificial o las plumas de insulina inteligentes para terapias con dosis m\u00faltiples de insulina (MDI), sigue habiendo una necesidad insatisfecha de mejorar el control de la glucemia en personas con diabetes tipo 1 y no se tienen en cuenta las diferencias de sexo. Las mujeres manifiestan sistem\u00e1ticamente mayores dificultades para controlar su glucemia debido a los cambios del ciclo menstrual, y existen diferencias en la regulaci\u00f3n de la glucosa entre mujeres y hombres sanos que no se traducen en tecnolog\u00eda.\n\nLa hip\u00f3tesis del proyecto es que la personalizaci\u00f3n de las terapias de insulina en funci\u00f3n del sexo se traducir\u00e1 en un mejor control de la glucemia, especialmente entre las mujeres, que podr\u00e1n contrarrestar mejor los efectos del ciclo menstrual. Entonces, el objetivo general de este proyecto es la personalizaci\u00f3n de la terapia de insulina seg\u00fan las diferencias fisiol\u00f3gicas en mujeres y hombres para un mejor control de la glucemia. Se har\u00e1 hincapi\u00e9 en el desarrollo de nuevos sistemas de apoyo a la toma de decisiones para la terapia con IDM\nque incorporen la monitorizaci\u00f3n en tiempo real de las fases del ciclo menstrual en las mujeres. Tambi\u00e9n se llevar\u00e1 a cabo una evaluaci\u00f3n de la necesidad de algoritmos de control espec\u00edficos para cada sexo para el p\u00e1ncreas artificial, as\u00ed como un estudio de simulaci\u00f3n de prueba de concepto que eval\u00fae una versi\u00f3n espec\u00edfica para cada sexo de nuestro p\u00e1ncreas artificial.\n\nPara alcanzar el objetivo y obtener los resultados tecnol\u00f3gicos deseados, en primer lugar, se generar\u00e1n conocimientos de vanguardia sobre la influencia de la fase del ciclo menstrual (folicular frente a l\u00fatea) en el uso del p\u00e1ncreas artificial y sobre el efecto gluc\u00e9mico del ejercicio y las comidas, las dos alteraciones m\u00e1s importantes en el control de la glucosa, gracias a 3 estudios cl\u00ednicos. En segundo lugar, se desarrollar\u00e1n nuevos modelos fisiol\u00f3gicos de ejercicio y comidas que incorporen los nuevos hallazgos. Estos modelos servir\u00e1n para desarrollar nuevas herramientas de simulaci\u00f3n capaces de describir las diferencias de sexo en el control de la glucemia, m\u00e1s all\u00e1 de los cambios de sensibilidad a la insulina entre fases. Se generar\u00e1n cohortes espec\u00edficas de pacientes virtuales que incorporar\u00e1n los modelos fisiol\u00f3gicos desarrollados en el proyecto, as\u00ed como escenarios espec\u00edficos para cada subpoblaci\u00f3n.
